Winner! Both of those settings were set to always, and as soon as I turned them off, the query times smoothed right out.

Is transparent huge pages turned off along with defrag? Check
/sys/kernel/mm/transparent_hugepage/enabled
and
/sys/kernel/mm/transparent_hugepage/defrag
Both should be configured with "never".
