Windows: Compiling and linking dynamically-loaded functions

within Section 34.9.6. of the PostgreSQL documentation (http://www.postgresql.org/docs/8.4/static/xfunc-c.html) there is an excellent summary how to compile and link extensions on a variaty of Unix and Unix-like operating systems.

How do I do the same on Windows, using Visual C Express ? Any hints or documentation URLs?
