Didn't see replies to this message, so...

Your query spends most of the time on sorting:

"        Sort Method: external merge  Disk: 21648kB"

and it doesn't fit in memory.
Try increasing work_mem somewhat to 50MB, you could do it for this particular connection only, if you don't want to
changeit for the whole server.
