Nice work, Peter.

Two minor suggestions:

(1) It's kinda unfortunate that the "SQL Commands" reference isn't that
obvious from the main TOC, as that's what a lot of people are going to
be coming to the documentation looking. Not sure the best way to fix
that, though.

(2) The Index, List of Figures and List of Examples take up a lot of
room on the main TOC -- is there a way to make these show up in a
separate page, and just include a link to that page from the main TOC?
