Create language PL/pgSQL failed

Create language PL/pgSQL failed

I want to using pl/pgsql in postgres, but failed to
create the language.

When I create a function written by pl/pgsql, met the
following error:

ERROR:  fmgr_info: function 21696: cache lookup failed

What happen to the system and how can I correctly
configure the language?
